I found my copy in Borders in Oxford Street (in London, UK). cost was 10 quid. The book is published by ellipse and a completely different shape and size to the original. It is about 12-14 cm tall I think, and 9 cm wide (I have not got it with me so I'm guessing here) and so is a lot thicker. Its printed on heavy paper, and the cover is very matt, but it does look like a low run book if you know what I mean. It includes a new introduction by Jon Savage (pants) and a new postscript by bill, which I really enjoyed reading, as he talks about a certain couple of young Austrians that he lent a copy of the manuscript to.. yodelling and big cleavage are also mentioned.
