Could a mistake happen this way? Absolutely. But not because you didn't follow the explicit directions. That would be because you were careless, and didn't check the address before you pushed the "Send" button. I would assume the "implicit" message in the DO NOT FORWARD direction is to be careful when you forward it. Either way, mistakes get made that an eraser won't/can't fix.
